Vulvodyni brukes som en samlebetengelse på smerter i underlivet hos kvinner. Det er en kronisk smertetilstand i vulva, uten smittsom eller anatomisk årsak som har vart i 3 mnd eller fler. Kvinner med vulvodyni opplever ofte en brennende, stikkende eller irriterende smerte i vulva, som kan variere fra mild til invalidiserende. Smerten kan være konstant eller episodisk, og den kan utløses av berøring, trykk, samleie, eller av å sitte over lengre perioder. 

 

Tilstanden er underdiagnostisert og ofte misforstått, noe som kan føre til betydelige fysiske, følelsesmessige og sosiale belastninger for de som er rammet.

 

Symptomene på vulvodyni varierer, men inkluderer vanligvis:

- Konstant eller periodevis smerte i vulva.
- Brennende, stikkende eller sviende følelse.
- Smerter under samleie.
- Ubehag ved bruk av tamponger eller ved gynekologiske undersøkelser.
- Smerter som forverres ved sitte i lengre perioder.

 

Diagnostisering av vulvodyni er utfordrende fordi det ikke finnes en spesifikk test for tilstanden. Diagnosen stilles vanligvis ved å utelukke andre mulige årsaker til smertene, som infeksjoner, hudsykdommer, eller andre gynekologiske tilstander. En grundig medisinsk historie, fysisk undersøkelse og eventuelle laboratorietester brukes for å eliminere andre diagnoser.

 

Årsakene til vulvodyni er ikke fullt ut forstått, men flere faktorer kan bidra til utviklingen av tilstanden:

- Nerveirritasjon eller skade: Noen forskere mener at vulvodyni kan skyldes irritasjon eller skade på nervene i vulva.
- Muskelspenninger: Økt spenning i bekkenbunnsmuskulaturen kan bidra til smertene.
- Inflammatoriske reaksjoner: Kronisk inflammasjon i vulva kan spille en rolle.
- Hormonelle faktorer: Endringer i hormonelle nivåer, spesielt østrogen, kan påvirke smertene.
- Genetisk: En arvelig komponent kan være til stede, noe som gjør enkelte kvinner mer utsatt.
- Psykososiale faktorer: Stress, angst og depresjon kan forverre smertene og påvirke livskvaliteten.

 

Behandlingen av vulvodyni er ofte sammensatt av flerebehandlinger og skreddersydd til den enkelte pasients behov. 

 

Vulvodyni er en kompleks og utfordrende tilstand som krever en tverrfaglig tilnærming for effektiv behandling. Tidlig diagnose og en individuell behandlingsplan kan forbedre livskvaliteten betydelig for de som er rammet. Samarbeid mellom leger, fysioterapeuter og psykologer er ofte nødvendig for å gi helhetlig omsorg og støtte til kvinner med vulvodyni.

 

En dilator kan være til hjelp for kvinner med vulvodyni, spesielt for de som opplever smerter ved penetrasjon, som under samleie eller ved bruk av tamponger. 

Dilatorer brukes som en del av en gradvis behandlingsregime for å øke kroppen sin toleranse for at det er noe der og kan bidra til å strekke og slappe av bekkenbunnsmusklene, samt redusere sensitiviteten i vulvaområdet. For mange kvinner med vulvodyni kan musklene i bekkenbunnen være stramme og smertefulle, og dilatortrening kan bidra til å redusere denne spenningen over tid.

 

Ved gradvis å introdusere dilatorer av ulike størrelser, kan sensitiviteten i vulva og vaginale området reduseres. Dette kan gjøre det mindre smertefullt å ha samleie eller bruke tamponger.

 

Kvinner som har smerter ved penetrasjon kan bruke dilatorer til å øve på å tolerere noe i skjeden. Dette kan forbedre komforten og redusere angst knyttet til seksuell aktivitet.

 

Hvordan gå frem:
- Start med den minste dilatoren i settet og arbeid deg opp til større størrelser etter hvert som komfortnivået øker. Det er viktig å ikke tvinge noe og å gå sakte frem.
- Bruk Smøremiddel:
- Bruk en vannbasert, uparfymert smøremiddel for å lette innføringen og gjøre opplevelsen mer komfortabel.
- Slapp av og pust:

 

Ta deg god tid. Hvis du føler smerte eller ubehag, stopp og prøv igjen senere.

 

Begynn med å bruke dilatoren i noen få minutter hver dag, og øk gradvis varigheten etter hvert som du blir mer komfortabel. Det er viktig å være konsekvent med treningen.

Kombiner bruken av dilatorer med bekkenbunnsøvelser, som Kegel-øvelser, for å styrke og slappe av muskulaturen i området.
